  &有子宫肌瘤的女性不少,多发子宫肌瘤的女性也不少,但有这么多、这么大子宫肌瘤的人真罕见。&医生说,吃药对缩小子宫肌瘤没什么帮助,手术是常用方法,但手术什么时候切、该不该切,要看有无症状、大小和所长部位。
  27颗子宫肌瘤切下来满满一盘
  一名肚大如箩、似乎已怀孕6个月左右的妇女,很腼腆地来到医院妇产科。她是来做产检吗?不是。她还没结婚呢!而且,她根本没怀孕!原来,这位女士今年39岁,在一家餐厅做服务员。她因为自己的肚子高高地隆起,被同事取笑说还没结婚就肚子那么大,是不是不正常。她自己用手指戳了戳,这肚子还硬梆梆的,不像长胖了。于是,她在同事的建议下挂了妇产科的号。
  检查后,她被确诊是子宫肌瘤导致的腹部鼓胀,不久后,她便躺上了手术台,做子宫肌瘤切除手术。
  &你能想象吗?两小时的手术切下来的子宫肌瘤装了满满的一盘,大大小小的子宫肌瘤有27个!&解放军458医院妇产科主任万兰主任医师告诉新快报记者,因为对方还未生育过,因此,保留了她的子宫。万兰推测,这位妇女的子宫肌瘤这么多、这么大,应该已有10年。
  得子宫肌瘤非人人需要手术
  万兰指出,目前对子宫肌瘤的病因了解仍然是比较肤浅,局部激素的失调也许是病因之一。
  不是所有子宫肌瘤的都能表现出一样的症状。长在子宫浆膜下、肌壁间的子宫肌瘤,可引起月经淋漓不尽,可继发贫血;长在压迫直肠位置的子宫肌瘤,可引起便秘;压迫膀胱的子宫肌瘤,引起小便不畅、不尽&&
  &不是每个人都需要手术,但若是有以上症状的肌瘤还是应该切除&。她指出,太大的肌瘤,譬如子宫像怀孕3个月了,也要考虑手术处理。此外,如果肌瘤有恶变的可能时,应考虑手术切除。&子宫肌瘤在怀孕期间有恶变的可能,但几率很小。&
  切不切,看大小、部位并权衡风险
  是否需要切除,还有一个肌瘤大小的标准。但是,目前对于多大的肌瘤应该考虑手术,还没有统一的认识。有人认为4cm以上就需要手术,有人认为7cm以上需要手术,对于小于4cm的肌瘤可以不处理。
  怀孕过程中子宫肌瘤可能会长大,因此,在准备生育前,若有长在子宫粘膜下、肌壁间、宫腔内的子宫肌瘤,会影响妊娠、受精卵着床,应将其切除后,术后半年~1年再怀孕;若子宫肌瘤长在浆膜下,不大,对妊娠无影响,大多数情况带瘤妊娠没有太大问题,则可先生育了再处理。没有生育要求的,只要子宫肌瘤小且无症状,完全可以不必处理。
  &切除子宫肌瘤,可以开腹进行,通过腹腔镜或宫腔镜进行,还可以通过阴道进行,近年来还有聚焦超声治疗和动脉栓塞治疗等新方法。&
