关于做好学年第一学期期末考试工作的通知
各部门、各单位：
为保证本学期期末考试工作顺利进行，切实做好考试命题、考务管理、试卷评阅、试卷分析、成绩录入、考试材料归档等各环节的工作，现就有关事宜通知如下：
一、考试安排与组织实施
1.根据学校教学工作安排，第18周（日-1月6日）为期末考试周。
2.各学院负责落实考试监考工作，要求监考教师严格履行监考员职责。
3.期末考试前，各学院须召开全体教师和学生会议，认真学习《关于做好学年第一学期期末考试工作的通知》《山东科技大学本专科课程考试管理规定》《山东科技大学考试违规认定及处理办法》《山东科技大学监考员守则》《山东科技大学考生守则》等有关文件，做好期末考试宣传、动员和思想教育工作。
4.校团委、各学院团委继续加强学风、考风教育，发放&诚信考试倡议书&，并做好&大学生诚信考试签名&工作等。
5.成立校、院两级考试巡视小组，全面监督检查期末考试工作，确保考试组织、考试秩序和考试纪律。教务处牵头成立学校考试巡视小组。各学院须成立本单位考试巡视小组，并于日前将巡视小组名单请报教务处，电子版发送至教务科邮箱。
6.教务处在校内网及宣传栏上设&考试违纪违规情况曝光栏&，对期末考试中发现的违纪问题在第一时间进行公告，并发布考试整体运行情况简报等。
二、考试规范要求
1.课程考试要求
（1）任课教师须严格审核期末考试学生的考试资格。重点审查学生平时作业完成情况、上课出勤情况、选修课报名情况及重修学生选课情况。按照学校有关规定，未完成作业或实验报告、缺课达三分之一（体育课达四分之一）和未选课的学生，不能参加课程考试。任课教师须在该课程考试前一周通知未取得考试资格的学生，并将名单报学生所在学院教科办，由学院汇总后报教务处备案。
（2）学生因故不能参加正常考试，必须事先提出书面申请，经学院同意，并报教务处批准后，可办理缓考。对无故缺考者，按旷考处理。
2.命题及试卷印刷
（1）考试命题应以教学大纲为依据，既能考核学生对基本概念、基本理论的掌握情况，又要注意考核学生分析问题和解决问题的能力。实验内容不单独考试的课程，试题中应有一定量的实验考核内容。
（2）考试命题要合理掌握深度广度，覆盖面要宽，要有代表性。题型科学，难易程度合理。题量适中，与考试时间相匹配。一个教学班的考试成绩应呈正态分布。
（3）课程考试由系（教研室）组织统一命题或任课教师单独命题。统考的公共基础课程，由系（教研室）统一命题，统一阅卷。必修课程须提供水平、份量相当的A、B两份试卷，选修课提供一份试卷即可。试题必须经过试做，并给出参考答案及评分标准。
（4）试题格式、阅卷要求及参考答案及评分标准等应严格按照《山东科技大学本专科课程考试管理规定》《山东科技大学关于考试命题的几点要求》执行。
（5）考试时间确定后，试卷必须提前一周送印。试卷须经系（教研室）主任和院（系、部）领导审核签字后，由命题教师本人交教务处教学科纸质1份，同时提交电子版。因送印不及时造成考试无法正常进行时，将追究相关人员的责任。
（6）考试用卷由教学科抽签决定。试题一经确定，不得变更。
（7）试题应严格保密，不允许以任何方式泄露试题内容。发现试题泄密时，须及时报告，并按有关规定做好应急处理。对当事人视具体情况作相应处理。
3.课程考核方式
（1）任课教师须提前向学生说明课程考核形式、课程总成绩评定办法，如平时成绩和结课考试成绩各占的比重、平时考核的具体要求等。
根据课程性质和特点，任课教师可采取灵活多样的考核方式，如课程论文、开卷等形式，但须保证公平、公正、客观、规范，且能够反映教学大纲要求和学生学习情况。采取课程论文、开卷等形式考核须经学院同意，并报教务处备案。
（2）课程闭卷考试时间一般为2小时。因特殊原因须调整考试时间，任课教师须提前申请，经教务处批准后方可执行。
4.阅卷及成绩评定
（1）凡统考、集体拟定的试题，阅卷评分工作也须集体进行，按照参考答案及评分标准，流水阅卷，阅卷后交替审查。
（2）阅卷评分标准应实事求是，宽严适当，公平合理。
（3）考试成绩记分制。考试课程成绩一般按百分制评定，60分（不含60分）以下为不及格；考查课、实习、课程设计等一般按五级记分制评定，即优、良、中、及格、不及格。
（4）总成绩的评定。考试课程成绩，以期末考试成绩为主，平时成绩为辅。平时成绩原则上不超过30%；考查课程成绩着重于平时成绩的积累，包括作业、实验报告、提问、小测验等。如平时成绩积累较多，能够准确反映学生的真实成绩，课程结束时不要求必须进行考试。
5.成绩提交及试卷管理
（1）阅卷完毕需将学生成绩提交到&教务管理信息系统&，并打印出成绩单一式两份，由任课教师签字后，送学生所在学院教科办。
（2）成绩提交系统后原则上不得更改。若因判卷或评分有误，应由任课教师说明情况，系（教研室）主任审核，主管教学副院长批准，并携带考生试卷到教务科办理成绩更改。
（3）学生对考试成绩存有异议时，允许学生提出书面申请查阅试卷。学生本人须填写《山东科技大学学生查阅试卷申请表》，经所在学院教学副院长批准后，报教务科组织查阅试卷。任课教师本人不得接待任何未经批准的试卷查阅。
（4）任课教师应对试卷进行统计分析，并写出考试成绩分析。考核不及格率超过20%的课程，任课教师须写出书面报告，说明原因，经学院分管副院长审核同意后，报教务处备案。
（5）日前，本学期全部课程成绩必须在网上提交完毕。逾期未提交成绩的课程及任课教师名单，教务处将在校园网上进行通报。各学院要通知并督促教师及时提交课程成绩，以便学生查询。
（6）学院要按照《关于规范课程考核试卷统一装订的通知》对试卷进行妥善整理、归类、保存。
6.补考工作
按照学分制改革要求，本学期考试不及格的课程（包括实习、课程设计等实践教学），学校将提供1次免费补考机会。补考命题形式与难度与正常一致（必修课程用备用卷，选修课程需再命一份试卷）。考试安排在下学期开学初进行，具体另行通知。
三、考试纪律要求
1.学生必须严格遵守《山东科技大学考场规则》，不得有作弊或违纪行为。发现作弊或违纪的学生，将严格按《山东科技大学考试违规认定及处理办法》处理。
2.学生考试一律在指定时间和考场内进行，并按座次表要求就座，考试中不得擅自调换座位。
3.学生必须携带本人有效证件（学生证或身份证，不可用其他证件替代）参加考试，并放在课桌的左上角，以备查验。无有效证件者一律不准参加考试。
4.每个考场必须配备两名或两名以上监考人员。监考人员必须佩带学校统一制作的监考证上岗。未经批准，教师不得找其他教师代替监考，严禁找研究生代替监考。一经发现将按照教学事故严肃处理。
5.监考人员要按时到场，认真履行监考职责。
（1）监考人员在考试前须提醒考生将携带的物品（除必要的文具外）放到指定位置，不得随身携带与考试相关的物品，不允许使用带存储功能的计算器，提醒考生若在考试期间随身携带手机或者其他有接收功能的电子产品，一经发现，按有关规定处理。
（2）监考人员不得在考场内接听电话、发送及阅读短信息、看书、看报或做其它与监考无关的事情。
6.违纪或作弊处理。监考老师发现学生有违纪或作弊行为，立刻终止学生考试，务必保留好现场证据材料，并在&考场记录单&中如实填写违纪、作弊情况，并要求学生在&考场记录单&上签字确认。学生如果拒签，要由两名监考老师签字确认。考试结束后监考教师要直接将&考场记录单&和证据材料送教务处，教务处核实有关材料后，确定违纪、作弊性质，并通报学生所在学院及学生工作处。
7.教师在阅卷过程中发现雷同卷或其他试卷异常时，应及时向学院和教务处报告，并协助学院、教务处进行核查，共同做出处理意见。
